ABSTRACT

Objective: To investigate the current practice of extracorporeal cardiopulmonary resuscitation (ECPR) for Chinese cardiac arrest patients after the publication of 2015 American Heart Association guidelines for cardiopulmonary resuscitation. Methods: A questionnaire was distributed to healthcare providers of emergency departments (EDs) and/or Intensive Care Units (ICUs) across 52 hospitals in China from August to November 2016. Data collection ended in February 2017. The questionnaire included three parts: (1) characteristics of the departments and the respondents; (2) knowledge about ECPR; (3) practice of ECPR in cardiac arrest patients (case volume, inclusion/exclusion criteria, ECPR procedure). The characteristics of the departments/hospitals were only answered by the head of the department. Results: A total of 1 952 (86.8%) respondents fulfilled the survey. Only 2.5% of the respondents from 3 of 52 hospitals performed ECPR. Among the three hospitals, the case number of ECPR were ≤5 per year and none of them had written ECPR procedures. Only one hospital had formal inclusion/exclusion criteria. The inclusion criteria included age between 18 to 60 years, suspected cardiogenic cardiac arrest, beginning of cardiopulmonary resuscitation 10 min. The top three reasons for the nonuse of ECPR were unknown fields (31.2%), potential ECMO-related side effects (26.9%) and cost (18.7%). Conclusions: ECPR for cardiac arrest patients are not well understood by healthcare providers in the emergency department or ICUs and its application is still in the early stage in China. Educational training and other interventions are needed to promote the clinical practice.

ABSTRACT

OBJECTIVE@#To investigate the level of serum microRNA-609 and its clinical prognostic value in patients with thalassemia.@*METHODS@#One hundred and twenty-seven patients with thalassemia treated in our hospital from April 2017 to April 2018 were selected, 100 healthy persons were selected as control group. The changes of miR-609 were analyzed by RT-PCR, the relationship between miR-609 and clinical indicators of thalassemia was analyzed, and the prognostic risk factors of thalassemia were evaluated by multivariate logistic regression analysis.@*RESULTS@#The relative expression level of miR-609 in thalassemia patients was 3.17±0.24, which was significantly higher than that in control group (P0.05). The incidence rate of mild anemia in high expression group was significantly lower than that in low expression group (P0.05). The number of patients with severe anemia in the miR-609 high expression group was higher than that in miR-609 low expression group (P<0.05). The incidence rate of dizziness, fatigue and fever in patients with miR-609 high expression group was significantly higher than those in patients with miR-609 low expression (P<0.05). There was no correlation between the level of miR-609 and the incidence rate of nausea in patients with thalassemia. ROC curve showed that the AUC value of microRNA-609 was 0.862, the sensitivity was 83.6%, and the specificity was 84.1%, which suggested that miR-609 had a high diagnostic value for thalassemia. Multivariate logistic regression analysis showed that MCH and mir-609 were risk factors for poor prognosis of thalassemia patients.@*CONCLUSION@#The increased level of serum miR-609 in patients with thalassemia is a risk factor for poor prognosis and can be used as a reference index for evaluating the efficacy for patients.

ABSTRACT

Objective To investigate the health care-seeking behaviors of Mosuo and Pumi people.Methods The subjects were enrolled by using the multi-stage stratified random sampling method and surveyed by the self-designed questionnaire.Results To tally 1669 subjects including 1121 Mosuo people and 548 Pumi people completed the survey.When Mosuo and Pumi people suffer from ailments,they preferred to buy drugs in drugstores(47.3% for Mosuo and 46.9% for Pumi),followed by visiting a local township hospital(27.0% for Mosuo and 24.3% for Pumi).When they suffered from severe diseases,they preferred to visit the county/city/state hospital(93.4% for Mosuo and 91.1% for Pumi).The mental disease were mainly treated in the county/city/state hospitals(49.3% for Mosuo and 52.7% for Pumi);notably,39.3% of the Mosuo respondents and 31.5% of the Pumi respondents skipped this question.Conclusion Health education,including awareness-raising activities on mental health,should be enhanced in Mosuo and Pumi people to further improve their health care-seeking behaviors.

ABSTRACT

Objective To investigate the association between polymorphisms of rs35753505, rs3924999in neuregulin-1 (NRG1) gene and the efficacy of risperidone after 12 weeks treatment in Han patients with schizophrenia from Yunnan of China.Methods A case-control study was conducted: 114 schizophrenic Han inpatients with 12 weeks single therapy of risperidone were randomly selected from July 2012 to March 2013 and 187 normal Han persons whose age and years of education matched the controls.TaqMan allelic genotyping technology was used to analyze NRG1 genotyping.Treatment effect of risperidone was evaluated by the positive and negative symptoms scale (PANSS), PSP, Raven’s Standard Progressive Matrices, Wechsler Intelligence Scale and Number Cancellation Test.Results (1) There was no statistically difference in genotypes, allele frequencies of rs35753505, rs3924999 polymorphic locibetween schizophrenic inpatients and normal persons. (2) The baseline clinical data of patients with schizophrenia in different NRG1 gene polymorphism was not significant. (3) The value difference of Number Cancellation Test One between pre and post treatment of risperidone was related with different genotypes of two polymorphismsin NRG1 gene, there was statistically difference in two genotypes of rs35753505 loci: G/A group was higher than G/G group (P=0.010) and in three genotypes of rs3924999 loci: A/A group was higher than A/G group (P=0.032) . (4) The value difference of Reven's Standard Progressive Matrices between pre and post treatment of risperidone was related with rs35753505 polymorphic loci: G/A group was higher than G/G group (P=0.004) (5) The result of correlation regression analysis between pre and post treatment of risperidone showed that the value difference of Number Cancellation Test Two was related with the baseline clinical data of course,dose,immediate memory,net score of Number Cancellation Test Two,Number Cancellation Test Three and the genotypes of rs35753505 loci.Conclusion After 12 weeks treatment of risperidone: the improvement degree of patients'attention directivity and concentrated force in different genotypes of rs35753505loci was different:G/A group was higher than G/G group.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the therapeutic and adverse effects of topotecan combined with cisplatin in the treatment of advanced squamous cell lung cancer and head and neck cancer.</p><p><b>METHODS</b>Totally 126 patients with advanced squamous cell lung cancer and head and neck cancer, which were confirmed by tissue pathology or cytopathology, were treated with intravenous infusion of topotecan at the dose of 0.75-1.2 mg/m2 (for 5 consecutive days) combined with intravenous infusion of cisplantin at 25-30 mg/m2 for 3 consecutive days. Each treatment course consisted of two 21-day cycles of the treatment.</p><p><b>RESULTS</b>No complete remission was achieved in these patients, and 61 patients had partial remission, 53 had clinically stabilized disease and 12 had progressive disease. The total response rate was 48.4% among the patients, with the median survival time of 10.1 months and one-year survival rate of 36.7%. The major adverse effects were bone marrow suppression and alopecia.</p><p><b>CONCLUSION</b>Topotecan combined with cisplatin may achieve a favorable response in patients with advanced squamous cell lung cancer and head and neck cancer, and causes tolerable adverse effect without accumulative toxicity.</p>

ABSTRACT

Objective To evaluate quantitatively the spasticity of elbow flexor biceps in stroke patients using integrated EMG (IEMG),and to correlate the IEMG with Ashworth scale.Methods The IEMG data was recorded from the biceps while passively stretch the elbow joint of a group of 90 subjects,and was correlated with the varying levels of Ashworth category(20 cases with grade 0,6 cases gradeⅠ,16 cases gradeⅠ~+,20 cases gradeⅡ,and 18 ca- ses gradeⅢ).Results The IEMG data were positively correlated with simultaneous Ashworth scale categories.The range of IEMG value corresponding to the Ashworth category is as follows:1.3~12.1 (6.7?5.4)?V?s vs Ash- worth grade 0;4.6~12.3 (8.5?3.9)?V?s vs Ashworth gradeⅠ.15.3~28.4(21.8?6.6)?V?s vs Ashworth gradeⅠ~+;37.2~68.9 (53.1?15.6)?V?s vs Ashworth gradeⅡ;82.3~144.1 (113.2?30.9)?V?s vs Ash- worth gradeⅢ.Conclusion The IEMG value can be used to quantify the spasticity of elbow flexor in stroke pa- tients.
